30 days before a Child's IFSP will expire, a progress review summary, known as a Form 13, is due. This form summarizes the child's progress on the IFSP outcomes. The Form 13 is completed directly on the EIMS website.
While there is no formal field for requesting service changes and/or discipline-specific assessments, you may add rows to the outcome review fields and include your clinical observations and justifications there.
Requests for changes or assessments must be submitted as soon as possible , preferably 30 days prior to the end date of the IFSP.
Practitioners need to keep track of their children's IFSP service page end dates to know when to submit these forms.
When you have entered your notes to fill in the boxes of "Outcomes" and "Narrative" or "Progress Report," you must click "Save." DO NOT CLICK FINALIZE!!
"Finalize" should be clicked only after the Agency Administrator reviews and approves it, and preferably only at the IFSP meeting. That is because it may not be changed or added to once "Finalized" is clicked. The finalize button creates the information to be imported into the IFSP document.
The Service Coordinator will have the information available if "Save" is clicked.

The service coordinator needs to "open up" the review process. Prior to that you will not be able to submit anything. Once the review process is "opened," the steps to complete the form on the EIMS is as follows:
1. Click on the "Early Intervention" tab in the child's file and select "IFSP Process" from the dropdown.
2. Under "IFSP Process" select "Progress Review Summary".
3. Select the {+} button next to the practitioner's name listed who is completing the Progress Review Summary.
4. View current Outcome(s) by clicking on the {+} next to the service.
5. Enter outcome and progress information in the Outcome Reviewed and Progress Report fields. Outcomes can be copy and pasted into the Outcome Reviewed field(s). Include Practitioner Name and date as an identifier of entry.
6. Select "Add Row" for additional Outcome Reviewed and Progress Report fields to be listed.
7. Review information entered and confirm inclusion of initials and date for any/all entries.
8. After you enter their information, you must click "SAVE" NOT "Finalize."
If you press finalize no edits can be made and that is not our policy, since A) We need to review them; and B) you may decide to add/change some information. Once finalize is clicked it cannot be undone and no other information can be added.
